Eligibility for the COVID-19 vaccination has been updated on Jan. 29, 2021 by health officials and now includes the following groups—healthcare workers, persons 75 and older, persons 65-74 years old, and *essential workers effective February 8, 2021. However, with the high demand for it in Madison County and an uncertain supply of vaccine, people in these additional groups should not anticipate being vaccinated for several weeks. 

If you fit into one of these categories and would like to receive the vaccine, please select and complete the appropriate online request form below. Your registration puts you on the waiting list for vaccination. It does not make an appointment for you. You will be called to schedule an appointment at our Community Vaccination Clinic at John Hunt Park. Please do not call the hospital or show up at the clinic without an appointment. We have received tens of thousands of requests and we are working through them as vaccines are sent to us. 

Thank you for understanding and for your patience as we serve you under the guidelines of state and federal health agencies. We will do our very best to get you vaccinated.

*Essential workers include firefighters, police, teachers and education staff, daycare workers, food and agricultural workers, manufacturing workers, U.S. postal service employees, judiciary, public transit workers, grocery store workers and clergy/ministers.
