Tax Information: Important Health Coverage Tax Document (1095-C Form)
According to new Federal requirements under the ACA Paperwork Reduction Act, you may receive a copy of your Form 1095-C upon request only. This means HH Health System is no longer required to furnish your Form 1095-C automatically. Click here to see the federal notice regarding this change.
