Health Science High School Instructor
To orient Health Science instructors to our facilities, the following document has been created:
Review the “Materials to Review & Complete” initially when starting, and then annually PRIOR TO beginning clinicals at our facilities. Keep the copies of completed forms/tests/affirmations with your records. If we need them for any reason, we will ask for them with the expectation that they can be provided the same day.

Undergraduate Nursing Student

All associated, completed forms/quizzes/affirmation statements must be submitted to your clinical instructor or their designee (ask your faculty for guidance if you are unsure.) THE STUDENT/CONTRACTOR ORIENTATION PACKET CONTAINS A QUIZ AND AFFIRMATION STATEMENT THAT MUST BE COMPLETED AND SIGNED, THEN GIVEN TO YOUR INSTRUCTOR TO BE MAINTAINED WITH YOUR DEPARTMENTAL RECORDS.
