Huntsville Hospital Health System is pleased to work with affiliated schools in meeting clinical rotation and preceptor needs. We are a teaching hospital and take pride in participating in the development of excellence in health care careers.

Proof of either COVID vaccination or exemption is no longer required. When obtaining a new Huntsville Hospital student or instructor badge, regulatory agencies require that we capture certain COVID vaccination information. Please complete the COVID-19 Documentation Form available through the link below and take the completed form along with your COVID vaccination card (if vaccinated) to Security when having your badge made.

In addition, the following Learning Modules need to be reviewed annually PRIOR TO your beginning clinicals at our facilities.  For instructors you must submit the associated tests and affirmation documents to your Dean or Program Director, prior to starting.   If you are a HHHS employee, you may either submit this education from your NetLearning Transcript, or submit these Learning Modules and the associated tests and affirmation documents to your school Dean or Program Director.

Review the “Materials to Review & Complete” initially when starting, and then annually PRIOR TO beginning clinicals at our facilities. Keep the copies of completed forms/tests/affirmations with your records. If we need them for any reason, we will ask for them with the expectation that they can be provided the same day.

Undergraduate Nursing Student

Review and complete the following materials annually, PRIOR TO beginning clinicals at our facilities. All associated, completed forms/quizzes/affirmation statements must be submitted to your clinical instructor or their designee (ask your faculty for guidance if you are unsure.) THE STUDENT/CONTRACTOR ORIENTATION PACKET CONTAINS A QUIZ AND AFFIRMATION STATEMENT THAT MUST BE COMPLETED AND SIGNED, THEN GIVEN TO YOUR INSTRUCTOR TO BE MAINTAINED WITH YOUR DEPARTMENTAL RECORDS.
